Despite the existence of safe and effective vaccines, rabies disease still causes an estimated 59, human deaths a year in the endemic areas in Asia and Africa. These numbers reflect severe drawbacks regarding the implementation of PrEP and PEP in endemic settings, such as lack of political will and low priority given to rabies. Infection is through a bite or by skin or mucous membrane contact with infected saliva. The period of the disorder can be as short as ten days or as long as one year; the usual time is one to two months.
Rabies is probably the oldest recorded infection of mankind. The development of the first rabies vaccine by Pasteur surely had been hoped to eliminate or at least drastically reduce its incidence. However, this goal has not been achieved because rabies is maintained in many animal reservoirs, including both domestic and wild. There are still many aspects of the pathogenicity of rabies that are unknown.
